Blood Tests To Identify Coagulation Markers

Part of your body’s natural clotting system includes a clot-dissolving process known as fibrinolysis. During this process, plasminogen is converted into active plasmin. Doppler Ultrasonography For Blood Flow Analysis

Ultrasonography uses sound waves that reflect off internal structures (for example, blood vessels) to generate an image. There are several types, including M-mode, two-dimensional, and Doppler ultrasound. This latter option is particularly useful for diagnosing a DVT. The sound waves reflect off red blood cells as they migrate with your bloodstream. This allows your doctor to identify the direction and speed of blood flow in your deep veins. Areas in which the velocity slows suggests venous compressibility. This, in turn, may suggest an obstruction caused by thrombi.

Doppler ultrasonography is noninvasive and usually performed with a handheld transducer. The high-frequency sound waves are emitted from the device, which is held against the surface of your skin near the site of the suspected clots. The transducer receives the reflected sound waves and transmits them to a computer. The computer compiles moving images based on the data.

Venography: The Gold Standard

If ultrasound fails to produce a conclusive diagnosis of a DVT, and your doctor’s suspicion of clots remains high, he or she may order an intravenous venogram. During this test, a contrast dye is injected into a vein of the affected leg. X-rays are taken to display the flow of the dye as it travels through your veins. These x-rays can produce clear images that show areas where the dye’s flow is slowed or impeded. Such restrictions indicate possible clots.

Venography has traditionally been considered the gold standard for diagnosing deep venous thrombosis. However, it is seldom used today. The contrast dye triggers an allergic reaction in a small percentage of patients. A helical CT scan uses x-rays to construct 3-dimensional images of the structures within your chest. A V/Q scan (sometimes called a lung perfusion scan) introduces radioactive material into your bloodstream and airways. Your doctor can use a special camera to follow the radioisotopes as they move. Areas in the blood vessels that reveal obstructed passages suggest the presence of clots.

Neither CT or V/Q scans are conclusive when diagnosing pulmonary embolism; results from both tests are based on factors that may be caused by unrelated conditions. For this reason, a firm diagnosis often requires pulmonary angiography.

Pulmonary Angiography For Diagnosing PE

During a pulmonary angiogram, a catheter is inserted in the groin and directed to the lung in which a blockage is suspected. The catheter is used to introduce a contrast dye directly into the pulmonary arteries. This dye helps your doctor visualize the arteries and pinpoint blockages. Similar to a V/Q scan, areas in which the flow of the dye is restricted or blocked suggest blood clots.

Pulmonary angiography is currently the most accurate test for diagnosing a pulmonary embolism. But it poses drawbacks. First, unlike the tests described earlier, a pulmonary angiogram is invasive, and thus accompanied by discomfort. Second, the contrast dye can potentially cause an irregular heart rhythm (though it is usually temporary) and kidney damage. Third, though rare, the blood vessel can be damaged during the procedure.

Another downside to pulmonary angiography is that the test requires specialized skill on the part of the doctor. Not all physicians are able to perform it.

The Path Of Migrating Nuvaring Blood Clots

Diagnosing Nuvaring And Pulmonary Embolism

On its own, the body will work to gradually shrink the clot, however, doctors still face the challenge of keeping a clot from moving upward in the bloodstream. Several factors can increase your risk of developing deep vein thrombosis. If you have a blood clotting disorder or have had a broken bone, you may be more at risk. Those who do have symptoms may have swelling in the upper or lower leg, with pain, warmth or redness in the affected area. However, these medicines do not actually reduce the size of the clots. Over time, the body will dissolve clots that currently exist.

Another method of keeping a clot from moving into the lungs is the insertion of a vena cava filter. This filter is inserted into a vein in the abdomen and keeps clots from moving upward. To prevent the formation of clots, patients may wear compression stockings on their legs to keep the blood from pooling in the lower extremities.
